Doctors' professionalism and sense of intrinsic reward replaced by crude economic calculation of value leads, unsurprisingly, to inferior service. The irony is that the DH now has a 'Social Enterprise Unit' to encourage the very ventures, such as the one he describes, it has crushed by central direction and 'one-size-fits-all' policy. And it's the patient that suffers.
